Currently, the Brazil Travelers Health Declaration Form (DSV) application is not available.
In response to the COVID-19 pandemic, Brazil has introduced its own Travel Health Declaration Form to help monitor the public health situation in the country.
The document is officially called the “Declaração de Saúde do Viajante” or DSV.
The Brazil Traveler’s Health Declaration Form is mandatory for all passengers wishing to travel to Brazil.
The form must be completed before arrival and presented to officials at border control.
Failure to show a completed form upon arrival will result in refused entry into the country.

What Is the Brazil Traveler’s Health Declaration Form?
The purpose of the DSV form is to help control the spread of Coronavirus.
This is done is by requiring each passenger to declare their current health condition at the time of departure.
Health authorities can use this information to reduce the possibility of foreign visitors carrying the virus into the country.
How Does the Brazil Travel Health Form Work?
The information collected in the DSV form is used to register the current health status of each traveler.
The form acts as a Travel Health Certificate, proving that the holder does not have symptoms of COVID-19 at the time of travel.
As part of the form, passengers must also agree to comply with the sanitary measures that the Brazilian authorities have in place in response to the pandemic.
By completing the form, you are stating that you will follow all the COVID-19 measures for the duration of your stay.
Who Needs a Traveler’s Health Declaration Form for Brazil?
All passengers traveling to Brazil need to complete the DSV.
It is a mandatory requirement for anyone wishing to enter the country by any mode of transport or entry point.
Without a correctly completed form, you will be automatically denied entry into the country.
Do Children Need to Complete the Form?

Brazil Health Declaration Form Requirements
Due to the pandemic, it is always necessary to check current coronavirus travel restrictions in Brazil before organizing travel.
If you have confirmed that you are able to enter the country with the current restrictions, then you can begin planning your trip.
This includes any necessary COVID-19 measures, such as completing the traveler’s health declaration form.
Before you start completing the form, you will need to ensure you have:
- A valid travel document
- Travel reservations
- An email address
- A phone number
As part of Brazil’s COVID-19 safety measures, all travelers over the age of 12 are also required to have one of the following:
- A negative COVID-19 PCR test, taken no more than 72 hours before travel
- A negative COVID-19 antigen test, taken within 24 hours of travel
How to Complete the Health Declaration Form Application Online
The Traveler’s Health Declaration Form for Brazil online registration can be completed quickly and easily. This is the only way to complete the form.
First, the system will ask you to register using your email address. This ensures that your details are registered correctly and that the completed form is sent to you by email.
Once you have registered, you will receive an email with a link to complete the form using the same email address.
The form will ask for personal details such as:
- Name
- Nationality
- Date of birth
- Travel document details
You will then need to provide information about your journey, such as your flight number and seat number.
For a faster process, you can complete online check in before starting the form, so that you already have your seat number.
The last part of the form requires you to answer some questions about your current health status. This includes if you have any coronavirus related symptoms.
You will then need to state that you have a negative PCR or antigen test result that follows Brazil’s testing requirements:
- PCR test within 72 hours of travel
- Antigen test within 24 hours of travel
The form must be completed within 72 hours of travel.
